Social Prescribing supports seniors in accessing non-clinical, community-based services to prevent or delay frailty by fostering resilience and social interaction using a comprehensive, strength-based approach. Participants of social prescribing programs have reported improvements in health and well-being, health-related performance, emotional state, social contacts, and day-to-day functioning. Working alongside a healthcare team, a community connector can offer more time to personally address each individual’s needs. Our areas of support may include help with benefit forms, access to meal programs, connection to in-home supports, community programming, and advocacy. The Connector listens carefully to the client’s story and then works with them to co-develop goals and a wellness plan to address the areas that matter most to them.
Our goal is to provide client-focused support to seniors in need, offering time to hear their stories and extend compassionate help where we can. We assist in their setting of achievable goals to improve their connection in the community and sense of overall well-being.
Our mission is to promote and assist prolonged independence and overall wellness of seniors in our community.

Eligibility
65+ years of age and older
Reside on Pender Island
Experiencing or at risk of frailty; social isolation and/or loneliness; barriers to accessing community-based resources; frequent trips to primary care
Referrals
Referrals can be from:
Self-referral
Friends, family, and community members with the clients' consent
Health care providers

Hot Lunch & Cafe
Hot Lunch: Happens every Monday at 12 noon with a few exceptions on a first come, first serve basis. Food delivery available.
Morning Café: The cafe runs from 9:30 am to 1:00 pm. Drop in for a coffee, tea, hot chocolate or snack and support from our technicians.

Mending: Mending operates from 10:00am to 12:00pm on the fourth Monday of each month.
