Please note: The TAP program is run by the Ministry of Health. If you have any questions regarding eligible services, please contact them directly (see link below). We can supply a TAP form to Pender-registered patients only, or to urgent or emergency care patients who require further care. Referrals are for the closest location for medical services that are not available in our community. For Pender Island residents, this is usually Victoria.

How does it work?

When you are referred to an eligible specialist or service, you may request a TAP form, preferably two weeks before travel. Do NOT leave it to the last minute! TAP forms may be picked up during office hours, 9 am to 12 pm and 2 pm to 5 pm.

Clinic staff will partially complete the form. If you require an escort (under 18 years of age or need someone to drive you for medical reasons), this box will be checked by staff. You must add your address, phone number, Personal Health Number, date of your appointment and signature, and your confirmation number (see next paragraph).

Well ahead of your travel date, you need to call the automated service at the number on the back of the TAP form (1-800-661-2668). Follow the prompts, including entering your Personal Health Number (PHN), date of appointment and other information. You will receive a confirmation number which you must write on the form. Present the completed TAP form at the ticket booth when returning to Pender
